We are looking to make the YHCR Interweave Portal available to care homes in HNY that have reached the appropriate level of digital and IT maturity. We’ve outlined our initial thinking on the requirements and use case
Why:
• Accurate meds reconciliation
• Overcomes challenge of incomplete admission information and discharge summaries when resident arrives in the care home
• View of appointments
• Access to the GP record especially problems/conditions and medications
• End-of-life information is very valuable for care home residents
Scope:
• The focus for this stage is to enable, where appropriate, care homes to be able to consume/view information from the YHCR. Data provision is out of scope currently and will be picked up at a later stage.
• It is anticipated that the first phase will provide care homes with access to the standalone YHCR Interweave Portal. However, it was agreed that any care homes with SystmOne may offer a quick win because of the pre-existing contextual launch capability from SystmOne to the Portal. This will be subject to unit type and will need investigation.
• It was agreed that we will target around 5 care homes for the initial pilot.
• ‘Entry level’ requirements for care homes to access the YHCR: ‘Silver’ level in Care Home Maturity Ladder (DSPT accredited, secure communications with partners (NHS mail), access to specific record/app (e.g. SCR, PKB)
• Assumption that the existing method of reporting a legitimate relationship with a patient is suitable for care homes
Access:
• Initial access via standalone portal, moving to contextual launch
• NHS/organisation email and 2-factor authentication via SMS to mobile/managed tablet – will this work for care homes?
• Authentication via SMS to personal mobile – need to investigate safeguarding issues and time impact
• Challenge that most care homes only have organisation NHS email NOT personal NHS or organisation emails. Issue for both set-up and ongoing support implications
• Other ideas discussed, for investigation on feasibility, support and cost:
o MFA for multi-users on a single device via PIN, fingerprint, facial recognition
o Tokens
o Contextual launch where possible/appropriate (depends on systems in use by care homes and their ability to support this. This route may significantly restrict the number of care homes who’d be able to get access
Users within the care home:
• Likely to be the senior or care home manager as well as registered clinical staff
• There will be no ‘one size fits all’ and will depend on the organisational size and structure
Other considerations:
• PIX (patient database) registration. Is this needed or are there alternatives?
• Can the PIX registration of large acutes, such as HUTH, YSFT and NLAG, be relied on and can any potential gaps (for example, for residents moving into the area to be near relatives) be accepted?

The Dreams team have provided a short list of potential care homes for the pilot and details of the software they use. It would be good to consider engagement with what appears to be the 2 market leaders to investigate potential for contextual launch, and we've asked for confirmation they are both on the new preferred supplier framework. As you say, the S1 homes can use the contextual launch.

Hi Tara, I guess we're waiting on further discovery with specific care homes before coming to a conclusion about pilot feasibility?
The issue with care workers having access to their phones for MFA, perhaps this is ok as staff will be 'sat at a computer' in an office, rather than roaming around the home with a laptop? I wonder how much of a barrier this actually is?
Dave has followed up with Ash @ TPP who has confirmed that the Care Home module in S1 is called ‘S1 Mobile Working Solution’ and as long as that’s the module the home has in place it seems safe to assume it should be able to support context launch of the Interweave portal.
