Ability to see where an ambulance journey went to

As a Portal user, when Transfer of Care occurs, I need to see where my patient was transported to, in order that I can make appropriate decisions such as contacting the destination, and/or communicate to relatives.

Other portal users such as mental health, GP and their staff, social care etc. often have to deal with relatives, carers etc and need to know the outcome of the ambulance encounter so they can continue to progress their enquiries efficiently
