ReSPECT forms visibility in YHCR cross-sector.

As a paramedic working in urgent and emergency care, the thought of having access to a patients care wishes immediately at your fingertips once on scene sounds a great idea. This would help by removing much of the ambiguity that lack of information can bring during a palliative/EOL attendence allowing for a more patient focussed approach in many cases. I just hope that information offered would be comprehensive as in my experience some ReSPECT forms are minimalistic to say the least which, if anything, increases ambiguity relating to apppropriate, taylored and best interest care. All in all a great idea in my opinion.

As a Care coordinator in primary care, I would support having the ReSPECT form care plan being integrated for YHCR, it would be beneficial for all patients to know that their wishes are shared between organisations and enhance individual patient care.

My Personal wishes part is a great idea and will allow again to understand what each person would like towards the end of their journey offering services to ensure the holistic touch is added and managed appropriately.

I would also like to suggest that 'my personal wishes' document/ care plan developed across South Yorkshire is also available to further enhance the care of patients.

Hello, my names Matthew and I currently work as a paramedic for Yorkshire Ambulance Service with a keen interest in palliative and EOL care as one the champions in North.

I would strongly support the ReSPECT form and advance care plan information being more readily available on YHCR as I have found delays in accessing this information has had direct impact on patients care.

As for the My personal wishes document, I think this is a fantastic idea and can help create a picture that allows for a holistic approach to managing the patient.

I would like to strongly support having Respect as a visible document in the YHCR shared record.

As a working GP and Palliative care lead for Sheffield Place ICB this feels like the single most useful document to be able to view (appreciate others will have a different view). The ability for YAS and hospital to be able to view and carry out the wishes of people is paramount to reducing hospital admissions. Respect being visible would be transformative and would increase the 'buy in' to the usefulness of the Shared record across services. I would also support Michelle's suggestion of having the 'My personal wishes' document alongside Respect which is important to communicate wishes of people who do not have the capacity to do so themselves.

I would also like to suggest that 'my personal wishes' document/ care plan developed across South Yorkshire is also available to enhance the care of patients that is being offered that meets the needs of patients, carers and their families.

I agree that this would be best as part of the bigger 'care planning' work in the portal. With ReSPECT plans currently sitting in so many different places and in so many formats, it needs a wider plan.

Unfortunately I've been unable to make contact with Robin to take this one forward.

However, we are currently in the process of discovery for introducing care planning to the portal more widely. I suspect we are best to mark this ticket as 'Future consideration' for now, and bundle it in with the care planning deliverables as we validate the concept and progress toward development.

I will mark as future consideration, but do let me know if you have any objections.

How do you foresee the ReSPECT forms reaching the YHCR? Are they typically held in a particular place or system currently?

I'm trying to gauge whether this is development similar to the Blackpear PDF consumption project or a consideration for our document upload initiative.
